Community Health Needs Assessment & Evaluation

 Overview & History
  • The last Community Health Needs Assessment in Bedford City/County was conducted by the Bedford Community Health Foundation in 1999. 
  • This assessment will help identify policy, systems, and environmental issues that promote and/or discourage good nutrition and physical activity for families in the community.
  • It will lay the groundwork necessary to develop a Community Action Plan aimed at changing policy, systems, and environment to support and encourage good nutrition and physical activity in the county's families.
  • This project will utilize the CHANGE process which has been proven successful in other jurisdictions.
CHANGE Process
  • The Coalition has chosen five sectors to be surveyed.  They include the Community at Large sector, the Community Organization sector, Health Care sector, School sector, and Work Site sector.
  • Data will be gathered from each sector using focus groups, surveys, and photo/voice evidence
  • The data will be entered into the CHANGE system and weighted according to the following criteria: 
  • Impact
  • Magnitude
  • Seriousness
  • Feasibility
  • Consequence of inaction
Outcome
  • A community action plan will be created based on the prioritized and weighted data. 
  • Two policy or environment-related activities, one addressing nutrition and one addressing physical activity, will be implemented in the county.
Time Table
  • Needs assessment begins -- July 1, 2012
  • Needs assessment completed -- June 30, 2013
  • Action plan completed -- December 30, 2013
  • Nutrition activities implemented -- June 30, 2014
